Trust Income

If you, your spouse or dependent child received any income as the beneficiary of a trust, you must disclose the name of the trust and the trustee’s name and address. If more than $1,000 in gross income was received from a trust asset, you must disclose the individual asset, if known. Do not list the amount of income received or the value of the assets.
